The Northern Ohio Trauma System (NOTS) adult trauma field triage criteria guide Emergency Medical Services (EMS) in determining whether a critically injured adult patient should be transported by ground or air to an appropriate trauma center. The key factors include:

  • Destination: Does the patient require Level I Trauma Center services, or can a local hospital manage the injury?
  • Mechanism of Injury/Trauma Criteria: Severity and type of injury, assessed using physiologic (e.g., vital signs, Glasgow Coma Scale), anatomic (e.g., specific injuries), and mechanism-of-injury criteria (e.g., high-energy impact), as outlined in guidelines like the American College of Surgeons (ACS) Field Triage Guidelines.
  • Weather Conditions: Determines if air transport (helicopter) is feasible.
  • Mechanism of injury: motorcycle crash, fall victim, loss of consciousness etc
  • Nature of illness: e.g., stroke, heart attack, seizures etc
  • EMS protocols: our EMS providers follow our local medical control protocols when determining appropriate transport destinations
